For the Course_Overview page, error Select a on click page for this action button. Generate on click page does not bring up the autofilled page named Course_NewEdit where I can select Form Vertical. The other pages work correctly except for this page. Did I do something wrong on the Course_Overview page?

You probably have changed something to the overview page. Does your overviewpage still have the correct Platform "Web”? If you changed it to "Native”, you don't get the vertical layout.
Other option is that you have not selected pofile "Responsive”, but one of the others.
