What would happen to the application that's been using the license if we decided to not renew the subscription? Will it be totally unusable and will the data be lost? And if the app is deployed on-prem, what would be the effect?

If on prem the model would shut down after a couple of hours. In the cloud you probably could not log in anymore. But you still could download your database and files. And you probably would get a mail about the need to offboard your app.
